Consent for Login Access to MPM Phone App and Client Portal CP 1.3.1 (c)
This form enables a Participant/Nominated Representative to give signed consent for My Plan Manager to provide login access to our phone app and client portal, to view a Participant’s personal and NDIS Plan information to additional parties such as a Secondary Contact (your parent, a co-parent/guardians, grandparents, close friends etc.), Support Coordinators or others.
Only the NDIS Participant themselves or a legal guardian of an NDIS Participant may give this consent. This consent can be revoked at any time by the Participant/Nominated Representative.
